Disability Support Coordination
My name is Evelyn Clinton and I have worked in the health care industry for over 35 years. If you are not convinced that you have the best support coordinator who puts your interest FIRST, then it is time for a change.
As your support coordinator, I will get in touch with you on a regular basis, I will visit and get to know you and understand your needs and personal goals. Most of all, I will work with you to achieve your goals.
I will support and encourage your independence and do-ability and your rights to decision-making and problem-solving, allowing you to live in the community as a valued and respected member.
I will work with you to design a service plan that meets your needs and address any complex barriers and challenges that may arise. I will advocate on your behalf to ensure your voice is heard and you receive the support and assistance you need.

to assist you to get the right products to suit your lifestyle.
